Getting started

It begins with a call to (713) 597-5131 or a referral from a hospital or doctor. We confirm eligibility and coverage, then schedule the first home visit — usually within a few days, sooner right after a discharge.

The first home visit

A clinician comes to the home to review medications, take vitals, look at the home for safety and fall risks, and understand what the patient and family need. Nothing is rushed — it’s the patient’s own living room.

A shared care plan

We write a plan and share it with the patient, the family, and the patient’s regular doctors. Everyone works from the same page — what’s being treated, what’s changing, and who to call.

A visit schedule that fits

How often we come depends on the situation — weekly after a hospital stay, monthly for stable chronic care. The schedule flexes as things improve or change.

Reaching us between visits
